Ribbon that goes from outside to inside jacket?

    It would depend on the ribbon.

    The ribbon on the tunic in this photo is for the "Winterschlacht im Osten 1941/42" aka Eastern Front/Russian Front Medal.
    While on active duty, the ribbon would either be worn as part of the ribbon bar(as seen on the chest) or as the ribbon is shown - worn through the second buttonhole.

    As with ribbon bars, there was an order of precedence for wearing of ribbons through the second button hole.

    It is indeed the Ost Front Medaille. Hitler expected a victory before the 1941 winter. When his army started to loose more men due to weather conditions than in combat morale went down tremendously. To boost this up and to acknowledge the sacrifices made during theis terrible winter , the Ostfrotn veterans were awarded this medal. It is quite common because millions were awarded ans the medal is worth about 25$ only.
